PRACTICE NURSE – CHRONIC DISEASE MANAGEMENT PREVENTATIVE HEALTH
We are currently seeking a Part Time – Chronic Disease Management Nurse to join its busy team, flexible hours, Full or Part time and state of the art premises in a tranquil Perth Hills location.
Requirements of the role include:
- Current AHPRA registration
- Previous experience in Chronic Disease Management
- Excellent communication and interpersonal skills
- Ability to multi task and work effectively in a busy environment
- Previous experience using Best Practice medical software is desired
- Experience accurately documenting GP management plan’s and performing comprehensive Health assessments.
- Knowledge of MBS item numbers and billing cycles.
- Minimum 2 years’ experience in a similar role.

A very busy centre with a sister Practice at the base of the Hills, this role doesn’t come around very often as once your embedded in to our Mead Medical family you won’t want to leave.
The Opportunity
This role will give you the chance to truly contribute to the ongoing success of the Practice, your responsibilities will incorporate:
- Being the face of our organisation, working with our team to effectively manage all aspects of reception including; welcoming patients,
- Working within our call centre answering phone, booking appointments and managing emails plus any other administrative tasks required
- Professional and responsive customer service delivery, including liaison with patients, specialists and all other stakeholders
