Please note that any patients WITHOUT a valid Medicare Card are not able to be immunised in a General Practice, you will need to attend a “State Based” vaccination hub to obtain a vaccine.

ASTRA ZENECA VACCINATIONS

Mead Medical are currently running Astra Zeneca vaccination sessions for anyone over 60 years of age at both of our sites on the weekends, this is to ensure that we do not reduce the number of regular appointment spots during the week for our patients. Please visit the online eligibility checker to determine whether you meet the criteria to receive this vaccination. You can book online via HOTDOC or our website. Please ensure you have completed a consent form when you arrive for your vaccine.

If you are under 60 and have already had your first dose of Astra Zeneca, you will need to continue to have your second dose with the same brand – you can not swap to Pfizer or any other brand. 

Please book online via HOTDOC or call practice – long delays may be expected on phones due to high volume of incoming calls, your patience is appreciated.  To reduce delays please book online.

You can lodge your completed forms by emailing to covidready@meadmedical.com.au or handing in to reception prior or on the day of appointment.

  • You will be able to book your appointment online or by calling the practice once dates have been determined.
  • If you meet the eligibility criteria for adults with an underlying medical condition and you are NOT a regular patient of Mead Medical, you will need bring a letter from your regular GP confirming your health needs meet this criteria.
  • It is preferable that you book your 2nd dose on the day you receive your first dose
  • Your 2nd dose must be administered by the same practice as the 1st dose
  • You will receive a reminder for your 2nd dose via SMS – please make sure your mobile details are up to date

Documents required on attending your appointment:

  • Completed consent form
  • Letter from your regular GP if you are not a Mead patient confirming health condition to meet criteria

PATIENT INFORMATION

During times of community outbreak, all patients will be triaged before entering our Practice. Any patient’s with flu like symptoms must book a telehealth appointment with your GP prior to attending the practice, your GP will then determine whether a car consultation is appropriate and advise accordingly.

We as a practice would like to acknowledge our patients and their continued efforts in helping minimise viral infections in general. We understand that whilst our State is currently in a low risk Pandemic phase, this does not mean that we are able to let our guard down in ensuring it remains this way by remaining vigilant and cautious. It would be easy to say we are low risk and therefore no longer require screening of people presenting with symptoms however mild, however this is how other States continue to end up in predicaments that lead to shut down of businesses /lockdowns.

The new strains are fast spreading and highly contagious and as a result Mead Medical continues to triage all patients and any presentations of even mild symptoms with caution until we can properly rule out Covid-19. This means that you may be asked to wait in your car until the GP can carry out an assessment, and if deemed necessary get Covid tested by appropriate providers.

We understand this may not always be convenient but then neither is another Covid outbreak. Our patients and staff are our highest priority and by continuing this cautious approach we have been able to continue to operate and provide care during the past lockdowns.

THINGS YOU MAY NEED TO KNOW ON THE DAY OF YOUR VACCINE AT OUR CLINIC 

All of your consent paperwork and proof of eligibility if required should have been completed and on your patient file
Please remember to keep your distance from others waiting for their appointment
You will not be able to discuss any other clinical matters during this appointment due to the tight timelines during a vaccination clinic, please rebook if you have other clinical concerns
If you have attended with other family members you may go in together
Doctor will double check your wellness for todays immunisation and your documentation
Please ensure you have not had another immunisation two-week pre and post this COVID jab
Doctor will give you a colour coded card which will indicate how long you need to wait after you have had your jab
Please wait in the cubicle or room that the jab was given to you until your nominated time is up – then make your way to exit
Once your time is complete please exit following signage
IMPORTANT – please do not leave practice until you have checked out with reception,
Please call nursing if you are feeling unwell whilst still in the practice
Your vaccination record can be found in your MyHealth record
Nurses will provide you with a Post vaccination information sheet before you leave
