Aged Care Services
Mead Medical Aged Care Services has significant business processes and structures in place which are supported by sophisticated IT systems, in-house resources and experienced staff that focus on quality assurance and compliance, human resources, information technology, legal and finance in order to deliver a high level of services to aged care facilities and communities. We are an accredited Practice in line with the RACGP standards and quality improvement is deeply embedded within our practice.
Our GPs work as a team to provide around the clock seamless GP services to Aged care facilities, ensuring that our large pool of Doctors can ensure continuity of care at all times. During times of Doctor leave or illness a buddy system is used to guarantee ongoing care to their patients.

Model of Care
Demands for general practice services can only be expected to increase with increasing numbers of RACF residents whose healthcare needs are complex. The team model of care by using both GPs and CMA nurses offers are more comprehensive approach to care of residents within a RACF and enables timely responses to RACF residents’ needs and improved health outcomes. With the CMA nurse support, GPs are free to meet clinical needs and demands of residents and facilities. With our model of multiple GPs, we facilitate what we believe improved resident access to continuous care, even when unforeseen needs arise.
Comprehensive Medical Assessment Program (CMA)
Mead Medical will provide an experienced Nurse Practitioner who in consultation with the GP will provide an onsite Medical assessment (CMA) for all residents, ensuring a comprehensive approach to each individuals care, improving patient health outcomes and strengthening the relationship between GP, resident and residential facility.
Our staff come armed with laptop and clinical software which is used to ensure continuity of care between our nursing staff and GPs
After Hours Care
Our team of GPs work closely together to ensure that your residents have care at those times of the night when they may need help.
All GPs are contactable by phone during business hours and will be supported by the locum service. Our GPs prefer to speak directly with RACG nursing staff prior to any residents being sent off to hospital to ensure it is the best course of action.
We work one on one with each facility to work out the best approach and meet the needs of both residents and the facility management.
Caring from bedtime to breakfast
Preventative Health
Mead Medical takes a proactive approach to the care of residents and will prioritise each year the availability and facilitation of annual flu vaccines for residents as well as Pneumococcal vaccines, and any other preventative health activities to keep the residents well.
History of the resident’s vaccines are all kept on our clinical software and added to our recall and reminder system which allows us to proactively track the health status of each person.
Medication Management Reviews (MMR)
Mead Medical will work closely with the facilities nominated Pharmacist to ensure that each resident routinely has MMRs completed to ensure the safe management of residents medications, they are carefully monitored and assessed during periods of transition from hospital stays, new residents, changing health conditions and residents that may have co morbidities and complex health needs.
