Practice Nurse – Chronic Disease Management & Preventative Health

Chronic Disease Management Nurse 

We are currently seeking a Part Time – Chronic Disease Management Nurse to join its busy team, flexible hours, Full or Part time and state of the art premises in a tranquil Perth Hills location.

Requirements of the role include:

• Current AHPRA registration
• Previous experience in Chronic Disease Management
• Excellent communication and interpersonal skills
• Ability to multi task and work effectively in a busy environment
• Previous experience using Best Practice medical software is desired
• Experience accurately documenting GP management plan’s and performing comprehensive Health assessments. 
• Knowledge of MBS item numbers and billing cycles. 
• Minimum 2 years’ experience in a similar role.
