45-49 Year old’s Health Assessment
This health check targets our patients who, are at risk of developing a chronic disease.
A health assessment at this stage of life can assist patients to make the necessary lifestyle changes to prevent or delay the onset of chronic disease.
Chronic disease risk factors include, but are not limited to:
- lifestyle risk factors, such as smoking, physical inactivity, poor nutrition or alcohol use;
- biomedical risk factors, such as high cholesterol, high blood pressure, impaired glucose metabolism or excess weight; and
- a family history of a chronic disease
What’s involved?
A 30-minute appointment with a nurse followed by a standard appointment with your usual Doctor.
You may bring a friend or family member with you if you choose too.
Our nurse will:
- Take routine observations and measurements including an ECG.
- Update your demographics, immunisations, social and family history ensuring our records are accurate and up to date.
- Provide information regarding Advance Care planning.
- Perform an assessment of your mood
- Identify any health, safety, social or family issues.
Our Doctor will:
Review the information gathered by the CDM nurse and offer any direction or support required.
40-49 Year old’s Diabetic Risk Assessment
People aged 45 to 49 years (inclusive) who have had a health assessment may also receive a type 2 diabetes risk evaluation if they are at high risk of developing type 2 diabetes and meet the relevant eligibility criteria.
Pathology
There are no blood tests required for a Health Assessment. However, you will be asked to provide a urine sample on the day of your appointment.

- Putting you in touch with expert advice, quality information and support
- Discussing services which could help you stay healthy in the future.
- Offering personalised and practical advice on how to reduce your risk of illness.
75 years & older Annual Health Assessment
The aim of the annual Health Assessment is to prevent illness and to look at ways that we can offer or refer you for support to enable you to continue living independently and safely with an optimal quality of life.
What’s involved?
A 60-minute appointment with a nurse either at the surgery or in your own home. To be followed by a standard appointment with your usual Doctor.
You may have a friend or family member present if you choose too.
Our nurse will:
- Take routine observations and measurements
- Update your demographics, immunisations, social and family history ensuring our records are accurate and up to date.
- Provide information regarding Advance Care planning.
- Perform an assessment of your mood and cognition
- Identify health, safety, social and care issues.
- Where an assessment has been done previously, we have the opportunity to measure any changes.
Our Doctor will:
Review the information gathered by the CDM nurse and offer any direction or support required.
This is also an opportunity to review your medications and address any concerns you might have
