Jade Swarbrick – Pharmacist
Jade Swarbrick is an experienced GP pharmacist at Mead Medical, bringing a strong clinical background and a passion for patient-centred care within general practice.
Jade began her pharmacy career as a community pharmacist after graduating with a Master of Pharmacy in 2018. She became credentialed in 2021 and completed a Graduate Certificate in Clinical Pharmacy through Monash University in 2022. Keen to explore the breadth of pharmacy practice, Jade has worked across a diverse range of healthcare settings, including hospital pharmacy roles at Joondalup Hospital and Northam Hospital, community pharmacy, residential aged care facilities, and conducting Home Medicines Reviews. She has also contributed to pharmacy education through tutoring compounding laboratories at Curtin University and previously worked as an educational visitor with NPS MedicineWise.
Since early 2023, Jade has worked as a GP pharmacist at Mead Medical as part of the WA Primary Health Alliance non-dispensing pharmacists in general practice program. In this role, she has established herself as a key member of the clinical team, conducting medication reviews as part of chronic disease management and leading initiatives to improve quality use of medicines and patient outcomes.
Jade is highly regarded for her clinical expertise, collaborative approach, and commitment to improving patient care. She is passionate about the role of pharmacists within general practice and actively contributes to the broader community of practice, including mentoring pharmacists new to GP-based roles.
More recently, Jade has also joined the CoNeCT MHE (Complex Needs Coordination Team – Mental Health Expansion) as a Care Coordinator, working with patients who frequently present to emergency departments across Perth-wide hospital services. Through this role, she continues to demonstrate the value pharmacists bring to multidisciplinary care coordination within WA Health.
Across her roles, Jade has found her niche in delivering holistic, patient-centred care—optimising physical and mental health management while advocating for patients facing psychosocial barriers to accessing healthcare.
